What if I have to cancel an appointment? »
We request the courtesy of at least 24 hours notice for cancellations, as this is time we could effectively use to treat other patients. Cancellations with less than 24 hours notice may result in charges consistent with the time allotted for the visit.
What is Contra Costa Oncology's privacy policy? »
We take our patients' privacy very seriously, and are in full compliance with the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule that was finalized on August 14, 2002. For details please see the full HIPAA Privacy Rule, as well as Contra Costa Oncology's privacy policy.
Can family members attend my appointments? »
At your discretion, all adult family members planning to participate in your care and to help you make decisions are welcome to attend your appointments.
Can family members receive information about my care? »
In accordance with the HIPAA Privacy Rule and our office policy, we require you to sign a Confidential Communication Preference form and list those persons authorized to receive information regarding your care. Individuals not listed will not be permitted to receive or ask for such information without your written consent.
What items will I need for my appointment? »
  • History and physical form (completed)
  • Financial agreement (signed)
  • Insurance cards; Medicare, Medical, Blue Cross, PPO, HMO, etc.
  • All medical records available
  • A detailed list of medications or ALL medicine bottles you use
  • A copy of any advanced directives for health care (living will or durable power of attorney)
  • A signed Confidential Communication Preference form
  • A signed Notice of Privacy Practices Acknowledgement
What information will I need to provide to your office when I check in? »
Once your first appointment has been scheduled, a welcome packet will be mailed to you detailing the necessary documentation to bring to your appointment. As you check in for subsequent appointments, please make our receptionist aware of any recent laboratory or radiological studies you have completed. This will help to ensure that your medical chart is kept as accurate and updated as possible. Additionally, please let us know of any changes to your address, phone number, workplace, or insurance coverage.

How long will my appointment last? »
Upon your first visit, you can expect to be in our office for approximately 45 minutes to one hour. Follow-up appointments will be significantly shorter.
What are the office hours? »
Our office is open Monday through Friday from 9:00 a.m. - 12: 30 p.m. and from 2:00 p.m. - 5:00 p.m., with phone calls answered during these hours.
What if I need medical attention prior to my scheduled visit? »
If you have a medical concern or problem that cannot wait until your next visit, please call our office at (925) 939-9610. If you require attention, we will work you into our schedule that day.
What if I experience a problem after hours, or on a weekend or holiday? »
An answering service will answer your call after hours and on weekends and holidays. The answering service personnel will page the on-call physician, who will contact you as soon as possible. Rest assured that one of our physicians is on call at all times.
What if I have a life-threatening emergency? »
Please dial 911 for any life-threatening problem or condition.
What is Contra Costa Oncology's financial policy? »
Due to the many insurance plans available and their continual policy changes, we request that you become familiar with your personal coverage plan and with the benefits, services, and financial responsibilities of your health care.
How do I acquire an authorization? »
All non-emergency authorizations and referrals will be mailed to you so that you may personally make these appointments to best accommodate your schedule.
What foreign languages are spoken in the office? »
The following foreign languages are spoken in our office: Arabic, Dari, Hindi, Italian, Russian, Sign Language, Spanish, Ukrainian, Vietnamese, Visayan.
